  1. Open an account by visiting one of our experienced Bankers.
  2. Select a link below, enter your information on a pre-formatted form and print the documents.
  3. Mail the forms to the appropriate parties

Use this form to send the Account Closing letter to your current bank

Automatic Deposit Notice

Use this form to change your payroll, Social Security, deposits, etc. to FBHP

Automatic Debit - Change of Instruction

Use this form to direct your automatic debits to FBHP
